membership card which will accompany your instructional booklet.
For your convenience, we have arranged with our health service
partners to provide members with the names and locations of your
closest participating locations via our dedicated toll-free or website
information networks. Our Support team is are also
available, during normal business hours, to assist you in locating
additional providers in your area when you call the number, located in this booklet and on your new membership card.
Of course, you may also go to our website at anytime of day or night
to obtain this or other detailed information. NOTE: If your favorite
provider is not already registered in our program, you may nominate that
individual, clinic, or health care facility to our Support team
for follow-up action.
Please keep in mind that your membership is NOT INSURANCE, therefore it is very simple and easy to use. There are
no health restrictions, or endless paper work to complete. You can
use the card immediately and as many times as needed . When you
present your card to a participating provider, you will receive
a discount at the time of purchase or service. Since we know your
family's health is your number one priority, all your legal dependents
may use your membership card, or you may order additional cards
for family members in their names for a nominal processing and registration
fee by simply calling the Member Support team.
In order to maximize your savings, please take a moment to familiarize
yourself with the instructions in your membership booklet. The more informed you
are, the more often you will remember to take advantage of this
tremendous savings your new membership card offers you and your
family, and the more money you will save on these most important
